Office Hours

Office hours are Monday - Friday for day and evening appointments with occasional availability on Saturday, 9:00 AM - 12:00 PM.  Consultations are by appointment only.
 

During the initial phone contact, the first consultation appointment is scheduled within one or two weeks, unless there is a waiting list. Phone calls are returned within 48 hours.

 

Fees are payable at each session by check, venmo, or cash.  Credit cards are not accepted at this time.

Insurance
 

Dr. Starkman is an Anthem Blue Cross/Blue Shield provider.
Other insurances may be applied under an out-of-network coverage.  The office can provide a statement for reimbursement of payment from the insurance carrier.

Privacy Practices Form
 

A Notice of Privacy Practices is provided for your information.  Please read the notice, print it, sign page four and bring the form with you to the initial consultation. It is important that you are aware of your privacy rights regarding how personal health information is protected and exchanged.  Confidentiality is strictly maintained and any collaborative exchange of information on a client's behalf is provided with signed consent.
